    The latest proposed rule to reduce Medicaid reimbursement for this biennium, which runs through 6-30-15, is now posted at under the resources tab.  ISHA requests your thoughts and input.  Some of the provisions of LSA Document #13-422 that might apply to members are as follows:

    1.         Home health reimbursement is being reduced by 3%.

    2.         Outpatient hospital services are being reduced by 3%.

    3.         Various provisions regarding hearing aids are being
            modified as highlighted in the LSA Document #13-422

    The SSA is requesting comments on whether and how it should revise the criteria in its Listing of Impairments (listings) for evaluating hearing loss and disturbances of labyrinthine-vestibular function in adults and children.  Comments are accepted through October 29, 2013. 

    The SSA is inviting comments and suggestions from individuals who apply for or receive benefits from the agency; advocates and organizations that represent individuals who have hearing disorders or disturbances of labyrinthine-vestibular function, state agencies that make disability determinations for the SSA, experts in the evaluation of hearing disorders, researchers, and other members of the general public.

    You can make comments directly to the website or feel free to comment at the end of this blog. 

    Questions to consider: 

    ·         Do the rules for evaluating hearing loss or disturbances of labyrinthine-vestibular function contain technical language or jargon that is not clearly explained? If not clearly explained, what technical language or jargon needs further explanation?

    ·         Are the requirements for otological examinations and audiometric testing provided in §§2.00B and 102.00B clearly stated? If not clearly stated, what requirements need further clarification?

    ·         What types of testing should the agency consider when evaluating hearing loss in adults or children who cannot cooperate in behavioral testing?

    ·         Would it be helpful to add a sample audiogram that contains all the requirements necessary for evaluation of hearing loss in adults or children?

    ·         What word recognition tests other than the Hearing in Noise Test (HINT) or the Hearing in Noise Test-Children (HINT-C) should the agency consider when it evaluates hearing loss treated with cochlear implantation?

    ·         Should the SSA provide examples of medical reasons for a discrepancy between the speech reception threshold and the pure tone average?

    ·         Could the SSA improve clarity by replacing the phrase “disturbances in labyrinthine-vestibular function” with the phrase “disturbances of inner ear function”?

    ·         Rather than evaluating disturbances in labyrinthine-vestibular function in adults under the listings, would evaluating disturbances in labyrinthine-vestibular function using residual functional capacity improve the determination process?

    ·         Should the SSA continue to evaluate disturbances of labyrinthine-vestibular function under the Special Senses and Speech body system?

    ·         What else could the SSA do to make the rules for evaluating hearing or disturbances in labyrinthine-vestibular function easier to understand?

    ·         Would a different format make the rules easier to understand (for example, changing the grouping or ordering of sections; use of headings; paragraphing; use of diagrams; use of tables)?

    ·         Experts who study disability believe that many personal, environmental, educational, and social factors contribute in significant ways to the relationship between an individual's hearing ability and the ability to work. Rather than providing criteria for evaluating hearing loss in adults under the listings, should the SSA evaluate all hearing loss using residual functional capacity?

    Tuesday, September 24, 2013:  Continuing education is inspirational.  I agree with Socrates, Education is the kindling of a flame, not the filling of a vessel.” The perspectives presented at the 2013 ISHA Fall Conference caused me to reflect on the genetic component of prior and current patients on my caseload with newly informed eyes.  I do not take copious notes during seminars; instead I apply my full attention to the speaker’s content and just jot down a few “golden nuggets” of information.  Below are the nuggets from my memo pad:


    §  When presented with a patient with hearing loss, consider it as a symptom not the diagnosis.


    §  Success in life is measured by happiness.


    §  Genetic Testing is not the same thing as a Genetic Evaluation.  Genetic evaluation is much more in-depth. 


    §  Gene expression can change. 


    §  When considering genetics, you must look at several generations of the family, not just the patient in isolation.


    §  “Genetics is not destiny.”  Just information to help family support child in reaching their potential.


    §  Between the 3rd and 6th month brain development is the greatest – 240%.  This is why early hearing detection is so important.


    §  40-60% of admissions to children’s hospitals are for genetic or genetic related conditions.


    §  To find a genetic counselor locally


    §  To find a genetic counselor nationally   


    §  Families can directly contact a genetic counselor if pediatrician does not share their urgency. 


    §  Don’t be hesitant to refer on to genetic counselor, developmental pediatrician, or multi-disciplinary medical team.


    §  Good resource


    §  Stuttering has a suspected genetic link – incidence with first degree relative = 20-74% vs. incidence in general population = <4.2%


    §  Genetic hearing loss is approx. 50-60% of all with hearing loss.


    §  Of genetic hearing loss 70% is non-syndromic.


    §  Genetic links between macrocephaly and Autism.

    The day concluded with the heartfelt sharing of three women about their experiences in living with genetic conditions.  It helped to hear their perspective as clinicians, so we could increase our respect for their journey.

    The new year of ISHA programs and initiatives has begun.  Registration for the Fall Conference on September 21 is open with Dr. Luis Escobar and others presenting information concerning genetic disorders.  Complete information and registration are under the events tab on the website.  The deadline for pre-registration rates is September 6. Note the new location for this conference at Kiwanis International on the northwest side of Indianapolis just off I-465 and Michigan Road.


    Membership renewal is also open for the new year on the website.  Your membership has to be renewed in order to get the members rate for the fall conference.  If you have any difficulties with the online registration process, please be sure to call Central Office for assistance. 


    The program committee (under the guidance of Kitty Kubat & Anu Subramanian) has been extremely busy securing speakers for the annual convention April 3-5, 2014.  They already have a program of recognized speakers on a variety of topics of interest so be sure to keep the date available.


    The Executive Council met on August 25, 2013 and some of the initiatives and concerns that we will be addressing through task forces are telepractice, caseloads in the school setting and guidelines for school settings.  We will also be monitoring other issues concerning changes in coding and any legislative issues that arise.  If you would be interested in working on these or other task forces, please be sure to let us know when you renew your membership or contact Central Office.  We need your help and input to address these issues in a way that will best benefit our membership.

    Several have contacted ASHA regarding the recent MedPAC report and recommendations to Congress. Attached is a letter that ASHA has written to Congress in response to MedPAC recommendations regarding a lowering of the cap threshold to $1,270 and completion of manual medical review for half of the services rendered to Medicare beneficiaries.

    • ASHA believes the recommendations would negatively impact Medicare beneficiaries and their access to medically necessary services. Overall, the report does not reflect the entirety of the discussion by the Commission regarding repeal and replacement of the therapy cap.
    • ASHA supports the increased attention to inappropriate high utilization, targeted geographic areas and individual providers and encourages MedPAC to consider measures that support predictive analytics and manual review focused on problem providers.
    • ASHA does not support the implementation of cost containment strategies as the sole solution to therapy payment reform, such as the implementation of the Multiple Procedure Payment Reduction (MPPR) for therapy services or the reduction of the therapy cap amount.
    • ASHA maintains that the long-term reimbursement of therapy services must take into consideration the complexity and severity of the patient's condition for speech-language pathology services as opposed to a "one-size-fits-all" model for all therapy services, and proposes the use of the National Outcomes Measurement System as a tool for collecting functional outcomes for Medicare beneficiaries receiving speech-language pathology services.
    • ASHA requests that any new reimbursement policy take into consideration that speech-language pathology is a unique and distinct service from that provided by physical and occupational therapists.

    IIEPRC and EERC: What DO these acronyms stand for??

    I have previously shared information about these resource centers on Learning Connection, ISHA’s HearSay newsletter, and also briefly during our Business Meeting at Convention. I feel the information available from these resource centers is very valuable to special educators – including speech-language pathologists – so much so, that I’m sharing the information again through ISHA’s blog.

    I represent ISHA on the Indiana IEP Resource Center and the Effective Evaluation Resource Centers’ Advisory Boards (IIEPRC and EERC). I attended their annual meetings on 3/7/13. Please visit their websites!! They have excellent resources!

    Some information you may find helpful on the IIEPRC’s website ( writing measurable goals, progress monitoring, LRE resources, effective IEP processes, inclusive practices, continuing education opportunities, and so much more!! Be sure to access the January 2013 document titled, Statewide Assessment Resource Guide and Toolkit: Participation Decisions and Use of Accommodations for Students with Disabilities. There are “Accommodations to Consider for Instruction” and “Accommodations to Consider for Assessments” in the communication areas of Articulation, Fluency, Voice, and Language (in addition to other eligibility areas).

    Information/topics you may find helpful on the EERC’s website: ( continuing education opportunities (that overlap with the IIEPRC), Evaluation Process FAQ, resources for parents, preschool assessment and intervention, assessment of English Language Learners, educational evaluation reports, evaluation practices, examples of district procedure documents (e.g., evaluation procedures, move-in procedures, referral procedures, etc.), and a RtI Data team book (to name just a few). There are also resources titled: “FOR ILLUSTRATION AND DEMOSTRATION ONLY / FIDO” -- SLD Eligibility Evidence, Criteria, and Data Sources and SLD Certification forms. These were developed to support local/district level decisions for eligibility in the area of SLD. There was discussion at the Advisor Board meeting to develop FIDO forms in the eligibility area of LSI (as well as some other eligibility areas). I will be sure to pass along information about this resource once it’s developed.
